Description

SeniorBridge is seeking a Part-Time RN dedicated to delivering the highest quality in-home care to complex chronically ill seniors. This position will work up to 30 hours per week.

SeniorBridge is Humana's Private Pay home care business acquired in 2012. At SeniorBridge, we're passionate about helping families and loved ones find solutions that make life better. We provide concierge care management, private duty nursing, and personal care assistance services to seniors.

Responsibilities

As an RN Care Manager you will be responsible for the following and will work under the direction of the Home Care Supervisor.

Conducting admission visits for new clients and their family

Conducting assessments and re-assessments to identify problems and need for more or less services

Ensuring in-home care delivery is in accordance with each client's contractual program requirements and is high quality and complies with company -adopted evidence- based clinical practice guidelines

Develop a plan of care based upon the patient's therapy, and in collaboration with the clients Physician.

Provide effective and safe teaching for patient, caregivers and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals.

Thorough and timely documentation

Act as a liaison to families as well as with collateral clinicians, professionals & agencies.

Effectively communicate with clients, families, Physicians, Attorneys, etc. and all members of the multidisciplinary team

Work effectively with all departments to ensure the best delivery of service

Participate in case conferences with other disciplines providing care

Observes infection control practices and uses standard precautions

Participates in quality management/performance improvement activities

On-Call schedule may include Monday-Thursday 5PM-8:30AM and Friday 5:00PM-Monday 8:30AM.

Required Qualifications

Registered Nurse with a NY License, and no disciplinary action

2 years of experience of in home case/care management

Experience working with the adult population

Knowledge of community health and social service agencies and additional community resources

Exceptional communication and interpersonal skills with the ability to quickly build rapport

Ability to work with minimal supervision within the role and scope

Ability to use a variety of electronic information applications/software programs including electronic medical records

Effective computer skills and experience with Microsoft Word, Outlook, and Excel

This role is considered patient facing and is part of Humana At Home's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

We will require full Covid vaccination for this position.

Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Preferred Qualifications

BSN

3-5 years of in home assessment and care coordination experience

Bilingual - English, Spanish
